Abstract

Penyakit jantung rematik (PJR) adalah kelainan jantung yang terjadi sebagai komplikasi dari demam rematik. Penyakit katup progresif biasanya berkembang setelah satu episode demam rematik akut (DRA), meskipun biasanya hanya diidentifikasi pada 30-50% kasus. Demam rematik sendiri berawal dari infeksi yang disebabkan oleh Streptococcus grup A (SGA), yang kemudian memicu proses inflamasi autoimun pada jantung, sendi, jaringan subkutan, dan sistem saraf pusat. Gangguan pada jantung disebabkan akibat reaktivitas silang autoimun antara antigen bakteri dan jantung. Pasien dengan PJR pada umumnya tidak menyadari kondisi yang mereka alami, sehingga mayoritas pasien berobat ke dokter setelah PJR menjadi lebih berat dan telah menimbulkan komplikasi. Komplikasi PJR yang sering ditemukan adalah gagal jantung, aritmia, hipertensi pulmonal, stroke, kejadian emboli sistemik, endokarditis infeksi, dan komplikasi terkait kehamilan. Sampai saat ini, skrining dan penemuan kasus aktif hampir secara eksklusif dilakukan di arena penelitian dan difokuskan terutama pada anak-anak usia sekolah. Ekokardiografi aktif yang dilakukan untuk penemuan kasus penyakit ringan hingga sedang berperan penting untuk meningkatkan harapan hidup dan derajat kesehatan pasien dengan mendeteksi PJR pada tahap awalSkrining ini penting untuk menekan angka mortalitas dan morbiditas pasien yang diketahui sudah atau berisiko mengalami PJR.

Rheumatic heart disease (RHD) is a heart disorder that occurs as a complication of rheumatic fever. Progressive valvular disease usually develops after one episode of acute rheumatic fever (DRA), although it is usually only identified in 30-50% of cases. Rheumatic fever itself originates from an infection caused by group A Streptococcus (SGA), which then triggers an autoimmune inflammatory process in the heart, joints, subcutaneous tissue, and central nervous system. Disorders of the heart are caused by autoimmune cross-reactivity between bacterial and cardiac antigens. Patients with RHD are generally not aware of their condition, so the majority of patients go to the doctor after RHD becomes more severe and has caused complications. The most common complications of RHD are heart failure, arrhythmias, pulmonary hypertension, stroke, systemic embolic events, infectious endocarditis, and pregnancy-related complications. Until recently, screening and active case finding were almost exclusively carried out in the research arena and focused mainly on school-age children. Active echocardiography performed for finding cases of mild to moderate disease plays an important role in increasing patient life expectancy and health status by detecting RHD at an early stage.
